Help Improve Care in Our Community
Willapa Harbor Hospital is seeking patients and family members to serve on our Patient and Family Advisory Council (PFAC).
PFAC members partner with hospital leadership to improve the patient experience, quality of care, and communication.
What Is a PFAC?
A Patient and Family Advisory Council is a group of patients, family members, caregivers, and hospital staff who work together to:
- Improve quality and safety of care
- Enhance communication between patients and providers
- Review educational materials and policies
- Strengthen discharge planning and care transitions
- Ensure care reflects the needs of our community
Who Can Apply?
We are looking for individuals who:
- Received care at our hospital, clinic, or outpatient services (or supported a loved one) within the past 24 months
- Can share experiences respectfully and constructively
- Represent the diversity of our community
- Are interested in improving healthcare for others
What Is the Commitment?
- Quarterly meetings (approximately 1–2 hours)
- 2-year term (renewable once)
- Orientation and support provided
- Virtual participation may be available.
Scope and Limitations
- The PFAC is an advisory council only
- The PFAC does not have decision-making authority
- The PFAC is not a grievance or complaint resolution body
- Individual complaints should be directed to established hospital processes
Reporting and Communication
- PFAC discussions, recommendations, and activities are documented in meeting summaries
- Information is submitted to the Director of Patient Safety and Quality
- PFAC feedback is incorporated into Performance Improvement activities
- Relevant updates are shared with the Board of Directors through PFAC representatives
