Swing bed services is transitional care. Swing bed services bridge the gap between acute hospital care and a return to the home environment. 

Short Term Skilled Nursing – Transitional care for patients recovering from surgery, illness, or injury. Perfect for individuals who need additional care before returning home.

Faster Recovery – On-site rehabilitation services can help patients regain strength and skills.

Qualifying Criteria – Patients typically need to have been admitted to a hospital for at least three consecutive days and require daily skilled care or rehab. 

Continuity of Care – Patients can remain in a familiar hospital setting with the same staff.
